  1. when connected to DC adapter, it starts blinking then led is stable on
  2. when connected to PC usb port, it is initially recognized (and led light on) then it is no more there and led is off
  3. in which condition should i measure the Voltages? when connected to DC or PC?

You can connect to DC adapter to have a try.

point 1: 1.6V
point 2 and 3 : 0.0V
point 4 and 5 : 1.6-1.7V

as i did before, point 2-3-4-5 has a polarity to be used as reference (a sign plus on one side).

one thing, led light on only a couple of seconds after plugging usb input. measurements collected during led light off

The result is abnormal.You can have a try again.I mean you can measure the voltage between these capacitances’ PINs like the picture below showing you.And tell me the value that you measure these five places,do not care the reference.

Every power is abnomally,there may be broken chip on the board.Can you ship it to us and let us repair?
